Admission Requirements (New Students & Transferee)

1. Form 138 / Certificate of Transfer Credential
2. Good Moral from Previous School
3. PSA Birth Certificate
4. PSA Marriage Certificate (if Married)
5. Medical & Dental Certificate from School Clinic
6. Proof of Permanent Address (Brgy. Cert; Light / Water Bill)
7. 1x1 and 2x2 ID Picture (1 Copy Each)

Enrollment Requirements (Continuing Students)

1. Student Clearance
2. Approval to Enroll
3. Advising Form
